KS4 Haven Plus Transition Pathway

KS4 Haven Plus Transition Pathway Purpose: The pathway is intended to support a student reintegrate into a home school following a permanent exclusion. All students need to be on a mainstream school roll and following a careful induction facilitated by the Raedwald Trust assessment centre. Push and pull factors are identified and a bespoke support package is created.

Timescales & Support 

The intention is that pupils will be supported to access their mainstream school for up to 2 years, with 10 hours of agreed bespoke support each week, which could be increased to support individual needs. This may vary dependant on the pupil and ongoing assessment of their progress on the pathway. Raedwald Trust staff will help the pupil to access their mainstream school through a variety of interventions and techniques to support the wider school and family, such as:

  • Attendance at the start of the day – registration
  • Integration at social times
  • Teach on-site in separate room or in small group
  • Attending a hub/pastoral group
  • Supported into and during lessons
  • Identification of additional barriers including home circumstances/ school observations/ SEND/ Young Carers/ Financial hardship etc.

Curriculum 

The curriculum offer is the mainstream offer and if/when appropriate may be facilitated by the Raedwald Trust. In some cases, the Assessment Centre may identify areas of need that could be supported and delivered as an intervention during the placement. In this case, this would be offered as part of the bespoke package in accordance with the home-school SENCo.

If there are times when a pupil is unable to access the home-school curriculum offer, Raedwald staff will facilitate learning via:

  • The Raedwald Trust Readers for Life curriculum
  • PE Health and Well-being curriculum
  • Life Essentials curriculum

Assessment 

During the induction, both push and pull factors are identified for the students. A baseline is created using the anxiety toolkit, which is re-visited across placement and progress shared with both the home-school and parents/carers.

If/when the Assessment Centre identifies significant gaps, this will be fed back to the home-school SENCo who may direct intervention to be delivered by Raedwald staff as part of the pathway offer. These identified interventions will be assessed according to the APDR cycle, directed by the home-school and in some cases facilitated by Raedwald Trust staff and shared with the parents/carers.
